Add Forms To Quik!
Two Ways to Add Forms
You can request we add forms to our Public Library that will be available to every user. Or you can add forms to your Private Library that will only be available to you.
Private Library Forms
If you are a current Quik! customer and we don’t have the forms you need in our library, you can add forms for your private use. Forms generally cost $20 per fillable page. Our standard build cycle is 10 business days. We offer rush builds (2-4 days) or field customization for an additional $10 per page.

Step 1. Prepare an email letting us know your name and account number (or the name your account is under). Indicate that you want to add the form for your private use.

Step 2. Attach a PDF of the form(s) to the email.

Step 3. Send the email to forms@quikforms.com

We will send you back an estimate of the fees that will be charged to build your form(s). Then when we receive your approval of the estimate, we’ll have your form(s) ready to use within 10 business days.
Public Library Forms
Don’t see the forms you want in our library?
Use the form below to request the form you would like to see added to the Quik! Library.
As a service to our customers, we do not charge for forms from product companies (i.e. American Funds), clearing firms (i.e. Pershing), and other vendors for the financial industry. We don’t want any barriers to making sure you have the forms you need.
When it comes to our Library of Financial Forms:

We are willing to post any form a company has.

Some companies are not willing to let us post certain forms.

Some companies require approval before letting a user access their forms.
